2014 – 2015 Season Information:
You must register with USA Hockey before you can register with CAHC. If you have not already done so, please register with
USA Hockey here.
You must print out and complete a copy of the USA Hockey Consent to Treat/Medical History form, USA Hockey Code of Conduct form, and the CAHC Code of Conduct and Waiver & Participation Release Signature form for each player you are registering (please see the three attachments). Hard copies of the three forms must be submitted to CAHC per USA Hockey regulations before your player will be allowed to participate.

First time skater $125 (first time registered for hockey in lifetime regardless of division)

*A late fee of $75 will be charged to all returning skaters (Squirts, Girls, Pee Wees, Bantams, and Midgets) if the minimum payment (20%) is not received by July 1, 2014.
Refund Policy
CAHC registration refunds shall be given under the following schedule if fees have been paid in full and CAHC bookkeeper is notified in writing:
a. Skater withdraws prior to October 31: 100% refund
b. Skater withdraws after October 31 but prior to November 30: 66% refund
c. Skater withdraws after November 30 but prior to December 31: 33% refund
d. Skater withdraws after December 31: No refund
* **80% of the current registration fees must be paid before the skater can take the ice for the start of the season on
October 1, 2014***
